We ask for what is needed to mail the package.
The request form asks for the information needed to mail naloxone and manage delivery. It does not require an account, checkout, or eligibility interview.
What the request form asks for
The form asks for a mailing-label name, Oregon mailing address, quantity, and optional email address. The address is needed to mail the package and confirm Oregon delivery. Email is optional and should only be used for request updates, delivery questions, or support.
The form does not ask for insurance, ID, a prescription, age, required training completion, demographic information, or a required explanation of why naloxone is needed.
How order and survey data are separated
Shipping records contain the information needed to fulfill a request. Optional survey responses are stored separately from the shipping order so service feedback does not become part of the order record.
The survey is never required to receive naloxone.
How requests are managed
The service keeps limited operational records to manage delivery, returns, supply, and requests that need review. These records are used to operate the mail-order program, not to make people create accounts or explain why they need naloxone.
Retention and access
Shipping records are kept only for service operations, such as fulfillment, returns, reporting, and troubleshooting. Access is limited to people who need it to operate the service.
